Board Election Bio – Jennifer Ofelt

Jenn Ofelt has more than 10 years of post-acute care leadership experience including Medicare/Medicaid-certified home care, private duty home care, hospice, assisted living, adult day services, skilled nursing and long-term care.

She is currently the Vice President of Home Care at UnityPoint at Home. UnityPoint at Home provides Medicare/Medicaid-certified home care services to an average daily census of 4200 adult and pediatric patients from 14 locations in 3 states. In her current role, Jenn leads, directs and coordinates all clinical, financial and business operations for UPaH’s home health service line. She is responsible for ensuring delivery of high quality, cost effective clinical care for home health patient populations and facilitates the cascading of strategies and execution of action plans to facilitate growth and ensure operational and financial success.

Prior to joining UnityPoint at Home in 2016, she served as Executive Director of WesleyLife at Home, which provides home care, hospice, adult day center, private duty home care in 15 Iowa counties. Before relocating back to Iowa, she was the Home Care and Hospice Clinical Manager at Sanford Health, where she provided clinical leadership, including the development of policies and procedures and the oversight of quality assurance and performance improvement initiatives. Prior to moving to the Sioux Falls region, Jenn was a Nursing Supervisor at Good Shepherd Care Center in Mason City, IA, a 210-bed SNF/LTC facility with a secure memory care unit and an attached 46-apartment assisted living facility.

Jenn earned her Master of Science in Nursing and Master of Healthcare Administration from the University of Phoenix, a Bachelor of Science in Nursing degree from South Dakota State University and an Associate Degree in Nursing degree from North Iowa Area Community College.
